Service Agreement

This Service Agreement is made between Barbara's Answering Service with the mailing address of PO Box 1447 Claremont, CA. 91711.

I/We agree that the minimum service period should be 1 month starting on [date]. Written notice to cancel service must be given to Barbara's Answering Service 30 days in advance of termination date. I/We understand that the below listed rates are subject to change.

Liability: I/We agree that Barbara's Answering Service shall not be responsible or held liable for any loss or damage as a result of the operation of its service or the actions of its employees.

AS DISCUSSED: Services to be initiated: 24-hour call forwarding to live operator and/or voicemail service to gather caller information, then forward to the appropriate on-call person with the preferred delivery method.

Data Storage: Messages will be available on the web portal for viewing, PDF printing, and accessibility.

Charges: $78 monthly service fee includes 80 inbound calls and all services listed above. Additional inbound calls and services may incur additional charges. Service fees are due no later than the 15th of each month. Any account greater than two months overdue on payment is subject to having their account placed on "Stop Answer" until all charges are paid in full and the account is current.
